I always understood this was very short and adding it only makes sense if bought fresh and used within a few days..

I would not bare-root, keep a bit of the original soil and mix it through the soil before potting the tree.
The dosage... hm. Let's assume it is 1/4 tablespoon for an average size and base the dosage on the relative size of your pot (and now the question is: what is an average size? )

This could be a problem; there is no label or stamp with the useful dates..
