Help me find out what kind of tree this is and why are the leaves falling off

It is a Serrisa, where are you keeping it? how are you watering it? This is a non hardy species that does not like being moved so if you have just got then that could be the cause. You don't say what country you are in either. Iy is difficult to tell, but it looks a bit dry, and what is that on the surface?

I received it as a gift just last week I live in the USA north east when I stick a moisture meter in it the meter reads moist I don't know what's on the surface it looks like a moss.

it looks more like an azalea . it may just be dropping old leaves...needs to be outside tho. they can take cold just no wind

Well, it's an odd looking azalea in a cheap chinese pot. I stick by Serrissa, the shape of the leaves are right.

I cleaned it up took off all the dead leaves and put it under some water and under some new plant growing lights and it looks better already in just one day under the lights.
